Do I accept your insurance? Complete the form below and call your insurance provider and ask if I am an "in panel provider." They will need the information on the form to find out.
If I am NOT on your panel, ask if they reimburse for "out of panel providers." Most PPO insurances do. Many of my clients are in this situation. If so, I will be providing a "Superbill" and you will be able to get all or most of your fee reimbursed by the insurance provider.
